In Sickness & in Health

There is this one question married people know the answer to. When we promise someone in front of God or in front of a presiding person to stay in sickness & in health, do all of us mean it?

Someone asked me why people go in the midst of medical crisis. When a spouse turns very ill, the promise to hold on slowly fades away, to an indefinite blur.

Why?
